Comprehensive Care Facilities The Citizens Care and Rehabilitation Center is Frederick County's premier skilled nursing center.

Citizens Care and Rehabilitation Center is a 172 bed skilled nursing facility offering state of the art rehabilitative services. Its guiding principle is to provide outstanding and innovative healthcare in an environment that remembers the importance of the individual and family. The Center maintains a high rating on the Maryland Health Care Commission’s resident/family survey and is one of the top-10 facilities in the state of Maryland for resident outcomes, staff stability, and overall satisfaction ratings. In late FY23, Frederick County saw its Montevue Mission reimagined. A decision was made to transition from an institutional model through the Montevue Assisted Living facility, to an evidence-based community-wide aging-in-place model where more of Frederick County’s vulnerable low-income seniors can be supported where they are. Budget Highlights The FY25 budget reflects higher Medicaid, Medicare and Private rates and a 0.5% increase in occupancy for Citizens Care and Rehabilitation Center. Personnel and other operating costs were increased due to inflationary factors and salary increases.
