Comprehensive Care Facilities The Citizens Care and Rehabilitation Center is Frederick County's premier skilled nursing center.

Citizens Care and Rehabilitation Center is a 172 bed skilled nursing facility offering state of the art rehabilitative services. Its guiding principle is to provide outstanding and innovative healthcare in an environment that remembers the importance of the individual and family. The Center maintains a high rating on the Maryland Health Care Commission’s resident/family survey and is one of the top-10 facilities in the state of Maryland for resident outcomes, staff stability, and overall satisfaction ratings. Budget Highlights The FY26 budget reflects higher private pay rates, offset by a slight decrease in Medicaid rates as well as a 2% decrease in occupancy. The budget includes salary increases and an increase in operating costs due to inflationary factors.
